Choose what you need below. These are two separate forms — completing one does not complete the other. Most households using the pool will complete both: the Pool Access Form to get a key fob or app access, and the Pool Waiver that every adult must sign.

Key fob & mobile app access

Register for pool access — request a household key fob, an additional or replacement fob, and/or mobile app access. This is the form that gets you into the pool.

Swim-at-your-own-risk liability waiver

Sign the Tavola West Acknowledgment of Risk, Waiver, Indemnification and Release of Liability. Every adult (18+) must sign their own waiver; a parent or legal guardian signs for each minor. This does not grant access by itself.
